Male Living With A Uterus: 24-Year-Old Delhi Man Visits Doctor To Check Infertility, Discovers His Rare Disease PMDS

In a strange incident, a 24-year-old man discovered that he had a fully developed uterus after he came for an infertility evaluation. According to the doctor, the male patient had all the usual male physical characteristics and was born and raised as a male. However, during the examination, it was discovered that the patient had no testes on his scrotum. Upon testing his semen samples, it was found that the male did not produce sperm. Dr Susheel Kharbanda, Chief Urologist, RG Hospitals, Rajouri Garden termed the condition as azoospermia.

Upon further testing, it was found that he was chromosomally normal, i.e he had the usual XY chromosomes. ultrasound and MRI scans revealed the unexpected presence of a well-formed uterus, India Today reported.

Further tests revealed that the man had his testes developed inside his body, resembling ovaries. Doctors then carried out additional genetic and hormonal investigations to understand the rare condition. Doctors termed the condition as Persistent Mllerian Duct Syndrome, or PMDS.

What is Persistent Mllerian Duct Syndrome?

According to doctors, when Mllerian structures, including uterus and sometimes fallopian tubes, remain present in a person who has male chromosomes and typically male external genitalia, the condition is known as Persistent Mllerian Duct Syndrome.

In most cases, the Mllerian ducts disappear because of anti-Mllerian hormone, or AMH, produced by the testes. If AMH is not produced adequately or the body does not respond properly to it, these structures may remain in a male body.

How Would The Condition Cause Infertility?

The presence of undescended testes can have an signicant impact on male fertility. Sperm production requires the testes to remain at a temperature lower than the body's core temperature. When the testes remain inside the abdomen, sperm production can be affected. However, infertility in males, is not always caused by low sperm count. It can occur through several causes such as anatomical, hormonal, genetic or developmental.

If sperms are not present in the semen, doctors may assess whether sperm can be retrieved directly from the testes. Depending on the findings, assisted reproductive techniques such as IVF or ICSI may be considered.

Why did doctors remove both uterus and testes

to remove the uterus and both testes through a laparoscopic procedure, which is a minimally invasive surgical technique. The medical professionals first removed the uterus as it could be cancerous. After the removal of the uterus, the biopsy report showed very minimal signs of malignancy.

The doctors then suggested the patient to remove his testes as keeping them would ncrease the risk of cancer developing in the future.
