Self-Managed Abortion Among Young People: Young people often have to navigate the healthcare system due to an unintended pregnancy and find the experience complicated, overwhelming and daunting. Need for privacy, convenience, lack of awareness about legal options or the inaccessibility of the formal healthcare system are some of the evident factors that contribute to this choice. In any case, this unsupervised approach carries significant health risks.

As per experts, Medical abortion (MA) drugs are a safe and effective option under clinical guidance, but some are drawn to unsupervised self-managed abortion. Medical abortion without consultation or professional care leads to missing crucial medical precautions that are meant to make the process safe.

1. Self-Managed Abortion Has Potential Side Effects

A healthcare provider confirms how far a woman is in her pregnancy, as it is a vital factor for the safety of the pregnant person and the effectiveness of the medication. It is also important to get a medical opinion on dosage, potential side effects, and critical warning signs.

2. Unprescribed MA Drugs Might Pose Concerning Health Risks

At times, because of mistaken beliefs about regulations, some pharmacists do not regularly stock MA drugs. If safe options are not accessible, some young people may be driven to providers who are not properly trained or regulated, which poses a concerning health risk.

3. Serious Consequences For Young Women

Sometimes, unsupervised medical abortion can have serious consequences. Without medical guidance, especially young women, who are at the edge of this risky spectrum. An incomplete abortion, a failed procedure, an infection, a delay in seeking medical attention, a failure to recognise warning signs, a dosing error, and insufficient pain management can all result in complications.

These can become life-threatening if medical attention is not received. Fear or shame associated with the situation may prevent a young person from seeking necessary medical follow-up, which can worsen an already critical health outcome.

4. Clinical Protocols

It is an important difference to note that while the medications are safe within a medical framework, using them outside of recommended clinical protocols transforms an otherwise safe procedure into a dangerous one.

5. Awareness About Reproductive Health Choices

Enhancing awareness and expansion of access to safe and confidential services is important for young people to be fully aware of their reproductive health choices.

As Vithika Yadav from the Pratigya Foundation for Gender Equality and Safe Abortion emphasises, "Self-managed abortion is recognised as a safe option in early pregnancy and, for many young people in India, it can offer privacy, autonomy, and timely care. When stigma, cost, or fear of judgment stand in the way of clinic-based services, we know that informed self-management can be an important pathway. What young people need is accurate information, access to quality medicines, and supportive systems - not shame or silence.”

Lastly, it is our collective responsibility to ensure that no young person feels their only option is an unsafe one. We must work together to bridge the gaps in information and access, creating a supportive ecosystem where safe, legal, and confidential care is a reality for all.
